Our Commitment to Your Privacy
Mindfulness Mental Health Counseling PLLC is required by law to maintain the privacy of your protected health information ("PHI"), to provide you with this notice describing our legal duties and privacy practices, and to follow the terms currently in effect.
How We May Use and Disclose Your Health Information
- Treatment — to provide, coordinate, or manage your mental health care and related services.
- Payment — to bill and collect payment for services, including submitting claims to your insurance plan (Alma, Headway, or your carrier directly).
- Health Care Operations — for activities such as quality assessment, licensing, and business management.
- As Required by Law — including in response to a court order, or to report suspected abuse, neglect, or an imminent threat of serious harm, as required under applicable state law.
Except for the situations described above, we will obtain your written authorization before disclosing your psychotherapy notes or other PHI to any outside party.
Your Rights
- The right to request restrictions on certain uses and disclosures of your PHI.
- The right to receive confidential communications by an alternative means or at an alternative location.
- The right to inspect and obtain a copy of your PHI, with limited exceptions.
- The right to request an amendment to your PHI if you believe it is incomplete or inaccurate.
- The right to receive an accounting of certain disclosures of your PHI.
- The right to obtain a paper copy of this notice upon request.
- The right to file a complaint with us or with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated, without fear of retaliation.
Changes to This Notice
We reserve the right to change the terms of this notice and to make the new provisions effective for all PHI we maintain. The current notice will always be available on this page and at your request.
